Background:
Molar-Incisor Hypomineralization (MIH) is a common developmental enamel defect that presents clinical challenges due to post-eruptive breakdown (PEB), increased caries risk, and dental hypersensitivity. Full-coverage restorations, such as stainless steel crowns (SSCs), are often recommended for severely affected molars. However, current utilization patterns and provider comfort with SSCs—particularly adult-sized SSCs and the Hall technique—remain varied.

Objective:
To evaluate the current use of SSCs for hypoplastic permanent first molars with PEB among pediatric dental providers and to assess provider confidence based on training background, region, and clinical experience.

Methods:
A cross-sectional electronic survey was distributed to approximately 10,000 members of the American Academy of Pediatric Dentistry (AAPD) from March to April 2025. A total of 336 U.S.-based pediatric dentists and residents responded. The survey assessed comfort with assessing hypoplastic molars, use of adult SSCs, and use of the Hall technique. Descriptive statistics were used to summarize the results.

Results:
Most respondents (93.2%) reported being extremely or very confident in assessing hypoplastic first molars. Additionally, 79.5% reported moderate to high comfort in placing adult SSCs for MIH-related PEB. In contrast, only 29.5% expressed high comfort using the Hall technique. Regional responses were diverse, with the North Eastern AAPD district most represented (26.2%), followed by the Western and North Central districts.

Conclusion:
Pediatric dental providers across the U.S. demonstrate strong confidence in assessing and restoring hypoplastic molars using adult SSCs. However, there is greater variability and lower comfort with the Hall technique. As this study is descriptive, no inferential statistics were conducted. Further research is needed to evaluate outcomes and support clinical decision-making protocols for MIH-affected teeth.
