Purpose: The aim of this clinical study is to evaluate the impact of using bonding agents during sealant placement on first permanent molars, including assessing sealant retention, marginal integrity, and caries prevention.
Methods: This split-mouth study enrolled 170 children aged 4 to 10 years with fully erupted and caries-free permanent first molars. Two permanent first molars per child were isolated using an Isovac system and sealed. One molar received a bonding agent prior to sealant application, while the contralateral molar received only the sealant. Sealant retention, marginal integrity, and caries incidence were assessed at 6, 12, and 18 months post-placement. Descriptive statistics were used to analyze the data.
Results: The age of was 7.8 years, comprising 51% males and 49% females. At 12 months, sealant retention was 69% with bonding and 63% without, while marginal integrity was maintained in 69% of bonded teeth and 55% of unbonded teeth. At 18 months, retention rates were 71% with bonding and 60% without, and marginal integrity was maintained in 63% of bonded teeth and 48% of unbonded teeth. Notably, enamel hypo-mineralization developed in two teeth without bonded sealants at 18 months, while no such cases were observed in teeth with bonded sealants.
Conclusion: Sealants applied with bonding agents trended a higher percentage of full retention and marginal integrity compared to sealants applied without bonding. Longer-term follow-up is necessary to definitively establish the significance of bonding agent use.
