Objective: This study aims to evaluate and streamline the transition process for special health care needs (SHCN) patients from the pediatric practice to adult care through a structured research questionnaire. The goal is to assess the perspectives of patients, families, and providers regarding barriers to care, readiness for transition, and the effectiveness of existing support systems. The study seeks to provide evidence-based recommendations for quality improvement initiatives and policy changes to address this critical gap in dental care for patients with SHCN.
Methods: Using the EPIC electronic health record system, patients aged 16 years or older with SHCN were identified. A 12-question survey was developed and administered to parents and caregivers, capturing both quantitative and qualitative data on personalized care plans and communication.
Results: The survey results will provide insights into caregiver perspectives on the transition process, highlighting challenges such as limited availability of adult providers, lack of insurance coverage, and inadequate preparation. Effective transitions require structured planning, interprofessional collaboration, and increased patient and family education.
Conclusion: The study aims to provide evidence-based recommendations for quality improvement initiatives and policy changes to address the gap in dental care for SHCN patients. Implementing structured processes such as readiness assessments, individualized transition plans, and improved provider communication can enhance the transition from pediatric to adult dental care, ensuring continuity of care for this underserved population.
